Job Description SHIFTS ARE (12) HOURS.
9AM-9PM OR 9PM-9AM.
MUST WORK AT LEAST ONE SATURDAY OR SUNDAY, IN ADDITION TO WEEK DAYS.
IT IS 4 DAYS WORKED WITH 3 DAYS OFF WEEKLY.

Direct Support Professional FLSA Status:
Non-Exempt Basic Function Under the direct supervision of the QDDP, Program Manager and Registered Nurse and in accordance with established agency policies and procedures, principles of normalization and inclusion and licensure standards, the incumbent provides training and support to individuals served utilizing available resources to maximize their independence.
Specific Responsibilities 1.
Provides hands-on training and instruction to individuals in the following areas as appropriate:
dress, hygiene and grooming; meal preparation, shopping and nutrition; communication; money management; appropriate behavior; traveling within the community; housekeeping; fire safety; clothing purchase and maintenance; socialization; use of phone, clock, calendar and recreation and leisure.
2.
Completes documentation including but not limited to incident reports, communication logs, seizure records, and emergency evacuation drills.
3.
Completes daily documentation, including but not limited to, ISP data progress notes, Individual Program Plan data, behavior and health data, shift exchange, hourly house log and visitor log.
4.
Assists individuals with performing basic housekeeping duties.
5.
Follows proper procedures in reporting and takes appropriate precautions for the security and safety of the residence, its occupants, furnishings, and vehicles.
6.
Assists with medical appointments as needed.
7.
Administers medication as per medication administration training.
8.
Transports individuals to and/or from day programs and community activities.
9.
Refrain from using phone while working to have personal conversations, to watch movies, text, face time, play games and anything other than work.
10.
Performs other job related duties as assigned.
Internal Relationships Has regular contact with Program Manager, Qualified Developmental Disabilities Professionals, RN's and other Direct Support Professionals.
Qualifications The incumbent must possess a high school diploma or equivalent and previous experience working with persons with developmental disabilities is preferred.
The applicant must have a valid local driver's license, three (3) years of driving experience, and an acceptable driving record and criminal background investigation report according to My Own Place policy.
The incumbent must be able to transport individuals to and from day programs and various appointments.
Related Factors Must provide an annual medical statement, along with results of a PPD test or chest x-ray, completed by a licensed physician stating that the employee is free of communicable disease.
Must be able to transport individuals via public transportation and/or agency vehicle to community activities, day programs, work, etc.
Must be able to navigate steps, lift 60lbs, and participate in walking and exercising for periods of up to 45 minutes.
Must attend meetings and successfully complete all mandatory training sessions.
OSHA Classification I.
Good oral and written communication skills.
Able to be understood by people supported by the agency Must be able to work independently, with minimal supervision, and exercise good judgment.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
